Examples of chronic conditions that can be well managed in the home setting:
Diabetes
Heart Failure
COPD (Lung Conditions)
Hypertension (Blood Pressure)
Dementias
Thyroid conditions
Bowel and Bladder dysfunctions
Kidney Conditions
Sensory Deficits (Vision, Hearing, etc.)
Examples of acute ailments that can be treated on the spot:
- Exacerbations or complications of chronic conditions such as heart failure or diabetes
- Infections
- Minor wounds, abrasions
- Muscle/bone strains and sprains
- Skin conditions/rashes or infections
During visits a Nurse Practitioner is able to:
- Diagnose and treat common illnesses/ailments
- Prescribe/adjust/refill your prescriptions
- Arrange for same-day Prescription home delivery
- Manage/order home laboratory work and other diagnostic testing
- Home Coumadin (Warfarin) PT/INR monitoring
- Order home Xrays, ultrasounds, EKGs and other diagnostic tests
- Home sleep studies and CPAP/oxygen ordering
- Laboratory specimen collection (blood, urine, etc.) in the home
- Monitor the status of chronic conditions
- Vital sign monitoring (Blood Pressure, Heart Rate, Oxygenation)
- Perform minor procedures such as wound debridement and ear cleaning
- Collaborate with your specialists (example: Cardiologist, Neurologist, Dermatologist, etc.)
- Arrange for daily/weekly/monthly visits by a registered nurse in your home
- Order Physical/Occupational/Speech Therapy home visits
- Prescribe durable medical equipment (wheelchairs, blood sugar testing devices, etc)
- Administer injections (vaccines, b12, etc.)
- Driving Safety Evaluations
- Mental status Evaluations
Visits can be as minimal as every 6 months or as frequent as once per week, depending on your health/stability and your preferences
Visits are billed to your Medicare/Insurance/Medicaid in the same manner as a doctor’s office visit
